Preparing For Your First Visit
In order to make your first visit productive and informative, please take the time to read through this page and prepare your medical records. Plan to spend an hour in the office during which time we will review your medical history, Dr. Wheaton will perform a thorough examination, and then provide you with his diagnosis and recommended treatment plan. In order to assist him in understanding your full medical picture, please bring the following information with you to this visit:
- Completed Patient Forms (download a copy from the right sidebar)
- Current Insurance Information
- Copies of Any Current Reports (x-rays, MRIs, lab reports)
- Films (if available on either CD or the original films)
- List of any medications and supplements
Insurance Coverage
Payment may be made by cash, check, VISA, Mastercard, Discover or Care Credit. If you pay by check and the bank returns your check unpaid, a $20 fee will be assessed.
Care Credit is a “credit card” for medical expenses. This gives us the ability to offer you several payment options including 3, 6, 12, or 18 month interest-free plans, as well as, 24, 36, and 48 month extended-payment options at a fixed interest rate.
