x-ray / MRI / CT reports or scans. Although this is not necessary for Dr. Wheaton’s consultation with you, it is helpful to have any recent reports. Scans are not necessary or preferable in place of reports if not available at time of office visit.
Financial Info
There are many payment options: cash, check, CareCredit, HSA/FSA, and credit card. We require full payment at the time of service.
Dr. Wheaton does NOT participate with insurance companies, as Prolotherapy is considered a holistic, alternative and non-covered treatment. You will then be given all the necessary paperwork for you to submit these charges to your insurance for reimbursement.
Dr. Wheaton is NOT a Medicare / Medicaid provider (under the Social Security Act). Therefore, as required by Medicare, all Medicare patients must enter into a private contract with Dr. Wheaton. All financial options will be explained thoroughly at the time of the appointment before proceeding with potential treatment.
